A hillrom service technician was dispatched to evaluate the light system.It was determined that the handle adapter locking mechanism had malfunctioned, leading to the sterile handle falling.The handle adapter was replaced and the device is functioning as designed.The handle adapter will be returned to trumpf medical for further investigation.A follow-up report will be submitted if new information becomes available.
 
Event Description
During surgery, the sterile handle fell from its lock and landed on the instruments table.No injury reported.
