It was reported that when the patient presented for device change out procedure, the left ventricular (lv) lead was disconnected from the device.After the physician wiped down the pin, the distal tip of the lead wiring came loose from the rest of the pin.An attempt to put the lead back together was unsuccessful.The physician elected to use the other 3 electrodes on the lead and placed it back in the header.Programming changes were made and the system was functioning normal.Device notes were added to monitor lead function.The patient was stable.
