Event Description
It was reported that failure to halt ablation occurred.A maestro 4000 controller was selected for a atrial fibrillation procedure.Radio frequency (rf) failed to ceased upon release of foot pedal.Rf was ceased by manually overriding on the remote control by the attending cardiac scientist using the on/off button over three seconds after the pedal was released by the physician.The procedure was completed using the same device with no patient complications being reported.
