It was reported the threads on the tip of the inserter are stripped.No patient involvement or harm.Attempts have been made and additional information on the reported event is unavailable at this time.

Reported event was confirmed with product return.Visual inspection identified that the threaded tip was damaged and stripped.Damage was too severe for dimensional analysis.The strike plate exhibited strike marks consistent with use over a potential field age of approximately 4 years.No other damages were noted.As the reported event did not occur during a surgery, it is unknown if the adapter cap was used during previous procedures.Review of the device history records and receiving inspection records identified no deviations or anomalies during manufacturing.A definitive root cause cannot be determined with the information provided.If any further information is found which would change or alter any conclusions or information, a supplemental will be filed accordingly.Zimmer biomet will continue to monitor for trends.
