It was reported that during the surgery, the surgeon noticed that the chamber on the valve was not functional.

Additional information received reported that the shunt did not drain the lcr liquid and was changed to a small programmable shunt.
 
Event Description
Additional information received reported that there were no environmental/external/patient factors that may have led or contributed to the issue.The patient was fine.The physician changed the neonatal shunt with a small shunt.It was noted the issue had been detected before implantation.

Manufacturer Narrative
