The companion 2 driver will be returned to syncardia for evaluation.The results of the evaluation will be provided in a follow-up mdr.(b)(4).
 
Event Description
The customer, a syncardia authorized distributor, reported that the companion 2 driver exhibited a system malfunction alarm and a decrease in cardiac output.The customer also reported that the blood pressure of the patient dropped significantly.The patient was switched to a backup companion driver and cardiac output and blood pressures recovered immediately.
 
Manufacturer Narrative
The customer-reported system malfunction alarm and corresponding loss of vacuum was confirmed through review of the patient data file and reproduced during investigation testing.The root cause was determined to be a malfunction of the pressor senor board as it was incapable of maintaining its specified vacuum set point.Syncardia has a corrective and preventive action (capa) to address this issue.Syncardia has completed its evaluation of this complaint and is closing this file.(b)(4) follow-up report 1.
