Event Description
Powerglide midline placed in patient's right brachial vein.Access achieved successfully and verified via ultrasound, unable to deploy wire from catheter, so device discontinued.2nd attempt 1 inch from 1st, with different midline, but from same lot number, also achieved successfully.Nurse experienced difficulty deploying wire again, but was finally able to advance the catheter, despite difficulty deploying the wire.When advancing the catheter, the midline wire was noted to be looped onto itself when disconnected.
