Event Description
Medtronic received information that prior to the implant of this transcatheter bioprosthetic valve, it was reported that the valve did not fit into the delivery catheter system (dcs).The valve was protruding through the capsule near the paddle pockets and could not be loaded properly.The capsule broke near the paddle pockets due to the valve wanting to protrude through.The valve and dcs were not used and a new valve and dcs were opened.The first valve and dcs did not touch the patient.No adverse patient effects were reported.
