Event Description
Patient initiated complaint.It was reported that the patient experienced constant pain and recent test show that he have elevated metal ion levels in the blood.Patient is scheduled for a revision in april this year and he asking for a compensation.Doi: 2007.Dor: not revised.(unknown hip).

Event Description
After review of medical records, patient was revised to address mechanical loosening and pain due to recalled right total hip arthroplasty.Revision notes reported that the blood testing demonstrate elevated serum cobalt chromium.Upon going to the capsule, it was noted that there was a significant amount of metal stained tissue.The muscle and primary tendons were not damaged by the metallosis.There was some corrosion at the trunnion.Radiographically, the acetabular component appeared to be loose with radiolucent lines in all zones.Doi: (b)(6) 2007.Dor: (b)(6) 2018.Right hip.

Plaintiff fact sheet received.In addition to what was previously alleged.Pfs alleged fatigue, sciatica, plantar fasciitis, difficulty sleeping, metallosis, mental health problem, fear, anxiety and limited adl.
