Event Description
An attempt was made to use one telescope guide extension guide catheter (gec) and one resolute integrity drug eluting stent to treat a severely tortuous, mildly calcified lesion with 80% stenosis in the proximal right coronary artery (rca).The device was inspected with no issues noted.The device was prepped per ifu with no issues noted.Resistance was not encountered when advancing the device.Excessive force was not used during insertion/delivery.It was reported that resistance was encountered when the stent was pushed forward in the telescope gec.It was detailed that the stent got stuck and could not be pushed any further.The stent was removed and found to be damaged.A new stent was used however, resistance was still encountered.The telescope gec was removed and it was discovered that the proximal form of polymer on-ramp area was damaged.The patient is alive with no injury.
