Event Description
Information was received from a healthcare professional (hcp) via a manufacturer representative (rep) regarding a patient with an implantable neurostimulator (ins).It was reported that during implant the initial impedance check was fine at 417 ohms.After they were no longer on phone support, it was suspected that the resident doctor in the room unscrewed the set screws too far which caused them to fall out of the header block of the ins.When doing the final device impedance check outside of the patient pocket, they could not get traction on the set screws or tighten the bottom set screws for the bottom lead.The hcp could not find any loose set screws so a new ins was used and impedance was normal at 372 ohms.The case was finished without incident.The patient was doing well.No patient complications were reported as a result of this event.
