Event Description
A health care provider (hcp) reported via a manufacturer representative that the blade broke during a laryngomicrosurgery surgery.The physician supposed a possibility that the blade was bent inside.There was no delay in the procedure as a result of this event.The procedure was completed using a back-up device.There was no patient injury.

Event Description
On follow-up, it was reported that the device was being used normally.The device was used after opening the new package.There was no patient or staff impact.
