|
It was reported that the catheter adaptor (patient connector) of a uv-flash solution trans set (short) - (b)(6) was ¿idled¿ and could not connect to the titanium adapter.The nurse observed this while preparing the patient for peritoneal dialysis therapy when the patient visited the hospital for a periodic transfer set replacement.As a result, the transfer set was replaced.The titanium adapter was not alleged and was still in use.There was no report of patient injury or medical intervention associated with this event.No additional information is available.
