Event Description
The reporter indicated the surgeon implanted a 12.6mm vticm5_12.6 implantable collamer lens, -14.0/+3.0/088 (sphere/cylinder/axis), in the patients left eye (os) on (b)(6) 2020.The lens had low vaulting and remained implanted.The plan is to explant/exchange the lens at a later date.The cause of the event was unknown.

B5: the lens was explanted on (b)(6) 2020 and was exchanged for a longer lens.The lens exchange resolved the problem.Claim # (b)(4).

Additional information: h3-device evaluation: lens returned in a micro-centrifuge vial with moisture.Visual inspection found no visible damage to lens and residue on lens.(b)(4).
