The suspect device was returned to olympus and evaluated.The reported problem was confirmed.The evaluation determined the unit's power switch was broken and replacement with a new version was required.
 
Event Description
A user facility reported to olympus that the device power button was stuck in the on position.There was no patient injury or harm, associated with the problem, reported to olympus.
 
Manufacturer Narrative
This supplemental report is submitted to provide the results of the legal manufacturer¿s investigation and device history record (dhr) review.The dhr was reviewed for the subject device.No anomalies were noted and it was verified the device was manufactured in accordance with documented specifications and procedures.The legal manufacturer performed an investigation.A conclusive root cause was not identified.The legal manufacturer determined, based on the results of the device evaluation, that the following can be inferred: due to the age of the device, it is likely that the power switch was damaged because the amount of operating force and the number of times the power switch was applied exceeded the assumed value, since the product was a product before a power switch design change was implemented.Due to the age of the device, it is likely that the slide switch has been worn by repeated use.The high brightness mode is detected by the slider switch, and it is inferred that the detection operation became unstable due to the wear of the slide switch.
