Event Description
The reporter indicated that on (b)(6) 2020 a 13.2mm, vicm5_13.2, -6.50 diopter, implantable collamer lens tore/broke during injection/delivery into the right (od) eye.The reporter stated " lens got stuck in the plunger while injecting the lens and got torn on implantation.".There was patient contact but no patient injury.It was reported that on (b)(6) 2020 a replacement lens was implanted and the problem resolved.

Additional information: h3- device evaluation: lens was returned in a micro centrifuge vial with moisture and debris on the lens.Visual inspection found the optic and haptic torn with foreign debris on the lens.Claim# (b)(4).
