As reported: "when i inserted the screw after drilling as usual, a thin thread-shaped metal piece came out from the screw twice.I pulled it out, and when i looked at the screw, the thread was peeled off.I replaced it with another screw and could insert it safely.The angle of the drill was not bad.After that another screw entered as usual.".

The reported event not be confirmed, since the device was not returned.More detailed information about the complaint event as well as the affected device must be available in order to determine the root cause of the complaint event.However, based on complaint history review, some potential root causes are, but are not limited to: - an insertion angle of the locking screw superior to 30°.Indeed, any excesses in the angulation might lead to the non function of the locking mechanism and even the damage of the thread of the screw.- the insertion of the locking screw too close to the plate sides, causing metal on metal abrasion, resulting in the reported screw metal threads.A review of the device history for the reported lot did not indicate any abnormalities.No corrective actions are required at this time.A review of the labeling did not indicate any abnormalities.No indications of material, manufacturing or design related problems were found during the investigation.
