Event Description
The reporter indicated that a 13.2mm vicmo13.2 implantable collamer lens, diopter -9.0 was implanted into the patient's left eye (os) upside down.This occurred on (b)(6) 2020.Intraoperatively, the lens was replaced with an alternate lens and the problem is resolved.Reportedly, no patient injury occurred and the cause of the event is reported as unknown.

H3: device evaluation: lens was returned in a micro-centrifuge vial with moisture and residue on the lens.Visual inspection found the lens haptic torn and residue on the lens.Claim#: (b)(4).
