Event Description
It was reported the coupling between the battery and charger was very weak; there were no black squares.The issue was the same with a new charger.The nurse tried moving the battery to improve coupling, but the ins was very superficial, and the coupling was still zero.Communication with the clinician programmer and patient programmer could be established.The implantable neurostimulator (ins) was found to be 50% charged.X-ray to check orientation of the ins was advised.No patient symptoms were reported.The patient was implanted for dystonia.Additional information was received from a hcp.It was reported that the issue began around mid-september.An x-ray was performed and it was noticed that the ins was moved and flipped.The patient is scheduled for a surgical revision to replace the ins.
