This is filed to report contaminate on the mitraclip delivery catheter.It was reported that this was a mitraclip procedure to treat functional mitral regurgitation (mr) with a grade of 4.The steerable guide catheter (sgc) was inserted and when the mitraclip delivery catheter (cds) (00204u117) was advanced through the guide tip, something could be seen on the clip.The cds was retracted and removed.Upon removal of the cds, it was noted that there was a clear plastic, gelatinous piece hanging from the clip.It is unknown where the piece of plastic came from.There was no damage noted to the sgc hemostasis valve and no damage to the sgc soft tip.Two xtw clips were implanted, reducing mr to 1-2.There were no adverse patient effects and no clinically significant delay during the procedure.No additional information was provided.
