A physician reported that during a cataract extraction with intraocular lens (iol) implant procedure, the trailing haptic could not be implanted in the bag properly.There was also an opinion that the haptic may have been deformed.There was a possibility that this occurred because the patient's condition was avitreous eye and excessive myopia, but this had never occurred before.The patient will be sent to a (b)(6) hospital for treatment such as removal or re-implantation of the iol.The iol remains implanted.Additional information was provided by the surgeon that the iol was left in the eye with the haptic sticking out of the capsular bag, and the patient was sent to another hospital where the haptic was placed correctly in the capsular bag without any issues.Per the surgeon, there was no patient harm and the iol was not deformed.
