Event Description
On (b)(6) 2020, a 21mm sjm regent heart valve w/flex cuff was selected for implant.As the valve was pushed with force during implant, the leaflet became fractured.All fractured pieces were confirmed to be removed from the patient.It was reported that no instruments came in contact with the valve, the leaflet motion was assessed, and the valve was rotated during the procedure.A new unknown sized unknown manufacturer valve was successfully implanted.The physician alleged a clinically significant prolonged procedure time due to the valve exchange.No patient consequences were reported.
 
Manufacturer Narrative
The reported event of a dislodged and fractured leaflet was confirmed.No other damage was noted.The device history record was reviewed to ensure that each manufacturing and inspection operation was performed and the product met all specifications.While the root cause of the leaflet dislodgement could not be conclusively determined, there was no evidence of material defect in the carbon coating that may have caused or contributed to the dislodged leaflets or orifice damage.The damage may have been caused by some external force applied to the valve which overstressed the carbon material.
