The device have not been returned to olympus medical systems corp.(omsc) but were returned to ocsm for evaluation.The evaluation of the device by ocsm confirmed the following; the bending section rubber was damaged and there was a hole.There was leaked at the bending section rubber.The video connector was corroded by water leak.The exact cause of the reported event could not be conclusively determined at this time.If additional information becomes available, this report will be supplemented.

Event Description

Olympus inspected the device at the service department of olympus (b)(4) (ocsm) and found that the angulation control lever of the device did not move and the angulation lock function was not released due to deterioration of the angulation mechanism by rust.There was no report of patient injury associated with this event.

This supplemental report is being submitted to provide additional information.The exact cause of the reported event could not be conclusively determined.It was guessed that the cause of the similar complaint that the angulation was locked and could not be released was that the drum unit in the control section was corroded due to flooding from the place where the leak occurred.Omsc concluded that the reported event may have been caused by corrosion of the drum unit in the control section due to flooding from the leaked bending section rubber, based on similar complaint information and the device inspection result by olympus china sales & marketing.The leakage at the bending section rubber was caused by a broken bending tube.Device history record (dhr) review indicates that the product was manufactured and tested in accordance with all applicable procedures and met all final product release criteria.If additional information becomes available, this report will be supplemented.
