Event Description
|
It was reported to boston scientific corporation that an advantage fit system was implanted into the patient during a mid-uretheral sling procedure performed on (b)(6) 2020 to treat stress incontinence.According to the complainant, there was a resistance while trying to pass the needle.When the device was removed, it was noted that the needle was bent which made it not possible to place the other side of the sling.Another box of advantage fit had to be opened to use a new delivery device to place the other side of the sling, and it passed through successfully with no resistance.Cystoscopy was then performed and that was when a perforation was noticed.The physician re-passed the sling arm that perforated the bladder and a catheter was left in place for 2-3 days post surgery.The patient reportedly fully recovered.
