Event Description
|
This is filed to report clip jumping.It was reported that this was a mitraclip procedure to treat degenerative mitral regurgitation (mr) with a grade of 4.The clip delivery system (cds) was advanced to the left atrium; however, during the initial clip opening, there was resistance turning the arm positioner and a squeaky noise was heard.Then the clip jumped open.Therefore, the clip was re-locked and closed.The clip then opened as intended and the procedure continued as normal.The clip was deployed.One clip was implanted, reducing mr to 1.There were no adverse patient effects and no clinically significant delay in the procedure.No additional information was provided.

The device was not returned.A review of the lot history record revealed no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no similar complaints from the lot.All available information was investigated and without the device to analyze, a cause for the reported clip jumpiness and resistance on the arm positioner resulting in noise could not be determined.There is no indication of a product issue with respect to manufacture, design, or labeling.
