Event Description
Medtronic received information regarding a thermal therapy system being used for a neuro soft tissue ablation procedure.It was reported that intra-operatively, there was heat at the distal tip and the site swapped out the laser.The same behavior occurred so the site pulled back the catheter.The surgeon was working on s placements and the second side did the same thing.The site set a safety target to trip the laser to prevent the issue from occurring again.Non-medtronic bolts were used.There was damaged to both catheters.The procedure was completed and there was no reported impact to patient outcome.There was a reported delay to the procedure of twenty minutes due to this issue.

Medtronic received additional information that the case was completed using another catheter.It was unknown what the cause of the issue was.
