Event Description
The customer reported a ventilator was identified with a defective nav-ring.The device was not in clinical use at the time the issue was discovered.There was no patient or user harm reported.

G4:05mar2021.B4:13mar2021.The device was evaluated by the customer and philips field service engineer (fse) who confirmed the reported issue.The fse replaced the front bezel.Following the modification, the device was reported to fully meet specification and returned to use.No other anomalies were reported.Submission of a report does not constitute an admission that medical personnel, user facility, importer, distributor, manufacturer, or product caused or contributed to the event.

The exact context of when the issue was found is unknown.Additional information regarding the context was requested, however, no response was received from the contacts.It was reported that the device was not in clinical use at the time the issue was discovered.There was no patient or user harm reported.A front bezel was returned for analysis.Previous investigations identified the cause of the navigation ring failure is due to liquid ingress due to variability of the assembly process.
