Event Description
It was reported that an ablation procedure with an intellamap orion high resolution mapping catheter, an intellanav mifi oi catheter, a non-boston scientific coronary sinus (cs) catheter and a non-boston scientific fixed quad catheter and a non-boston scientific sheath was successfully completed.The activated clotting times (acts) were greater than 300.The procedure was unremarkable and the patient was discharged per normal facility protocol.Three to four days later, the patient went into a different emergency room (er) facility with shortness of breath and chest pain.A pulmonary embolism (pe) was diagnosed.Interventions performed were unknown.The patient had a positive outcome.The physician who performed the ablation did not attribute the pe to anything specific during the procedure.All products were disposed of after the initial procedure.
