|
It was reported that during unspecified surgery, with an r3 offset impactor, after confirming that there was no problem with the installation, the surgeons tried to remove the cup, but found that it was spinning.The welding part of the flexible structure had failed and some parts were missing.It was difficult to remove it from the cup because it was spinning and it was firmly fixed, so it took time.After removal, it was switched to a s+n straight handle and surgery was continued.After that, it was confirmed that the missing parts were lying on the floor of the operating room, they collected all the parts and confirmed that they matched the failed parts.There was 15mins surgical delay.It is unknown if there are remaining parts in the patient's body.

Results of investigation: the device, used in treatment, was not returned for evaluation.Therefore, product analysis could not be performed at this time.So the reported event could not be confirmed.A medical investigation was conducted and confirms this case reports that during the surgery, it was noted that the offset impactor was missing parts.The surgery was completed using the straight impactor with minimal delay, and the missing parts were discovered on the floor after the procedure.Per complaint details, there was no other complication reported.Therefore, no further clinical assessment is warranted.A complaint history review found related failures; this failure mode will be monitored for future complaints for any necessary corrective actions.A review of the risk management file revealed this failure mode was previously identified.This device is a reusable instrument that can be exposed to numerous surgeries; damage from repeated use can occur.Damage from prolonged use, misuse or rough handling are likely probable causes of the reported event.We recommend that all reusable instruments be routinely inspected for wear and damage and replaced as necessary.At this time, we have no reason to suspect that the product failed to meet any product specifications at the time of manufacture.Based on this investigation, the need for corrective action is not indicated.Without the actual product involved and/or device information, our investigation cannot proceed.If the device or new information is received in the future, this complaint can be re-opened.No further actions are being taken at this time.We consider this investigation closed.
