The device has not yet been returned, but when it is a device evaluation and investigation will follow.If additional information becomes available prior to that time, a supplemental report will be filed.
 
Event Description
As reported, the image on the evis exera ii video system center was freezing on the display monitor.According to the initial reporter, the processor was continuously cutting in and out, twice in mid-case.The customer changed the monitor cable, but the issue continued, so they changed to another unit.There was no patient harm or consequence reported as a result of this event.
 
Manufacturer Narrative
This supplemental report is being submitted to provide the results of the manufacturer¿s investigation.A review of the device history record (dhr) was conducted during this investigation.The review of the dhr did not find any abnormalities or anomalies identified during production.The device met all specifications upon release.Based on the results of the investigation and the age of the device, it is believed that the device failed due to aging deterioration of the parts on the dp and cp boards.Olympus will continue to monitor the field performance of this device.
