Event Description
|
It was reported that during use in a procedure, the fan of the light source system does not rotate, causing the machine to overheat and the black screen was automatically turned off three times which seriously affected the operation process.Also was reported that the light guide connector and camera connector were found rust under standard disinfection conditions.The procedure was finished with a competitor device.No patient injury or other complications were reported.

The reported device, used in treatment, was not returned to the designated complaint unit for independent evaluation, thus visual inspection and functional testing could not be performed.There was a relationship found between the subject device and the reported incident.A review of the customer-supplied photographs was performed and observed the light engine fan was not turning while the lamp was on.Another photo showed the device was not powered up while other devices were.There was a residue on the light guide and the light guide scope adapters.There appeared to be wear on a camera head port.The complaint was confirmed and the root cause was associated with component failure.Please refer to the instructions for use for recommendations on proper use of the device and potential troubleshooting methods to prevent future occurrence of the reported event.A complaint history review concluded this was a repeat issue.A review of the device history records showed there were no indications to suggest that the product did not meet manufacturing specification or would not be able to perform as intended.If the product associated with this event is returned at a future date, this evaluation will be reopened for investigation.Internal complaint reference: case-2021-00054256-1.
