The returned sample was inspected and found the negative umbrella not seated properly in the housing causing the ops valve to not suction during the event as air was leaking past the negative umbrella rather than suctioning blood.The retention sample was visually inspected with no anomalies noted on the device.All ops valves are subjected to a 100% leak test that undergoes 5 separate programs to test that each component in the valve is functioning properly.These units are also 100% visually inspected both during the leak testing step and during packaging.(b)(4).All available information has been placed on file in quality management for appropriate tracking, trending, and follow-up.

Event Description
|
The user facility reported to terumo cardiovascular that during cardiopulmonary bypass, the valve would not allow blood to pass through.Per facility, they tried clamping on either or both sides of the valve the blood was spurting through the umbrella valve, he was able to make it work properly after much manipulation.It was located on the aortic root vent line.No known consequence or impact to patient.There was 10-20cc of blood loss.The product was not changed out.The surgery was completed successfully.
