Event Description

It was reported to boston scientific corporation that a solyx blue sis system device was implanted into the patient during a solyx mid-urethral sling procedure performed on (b)(6) 2021 for incontinence treatment.After the implant surgery, the patient experienced urinary retention and was brought back in the operating room on (b)(6) 2021 to release the sling.A cystoscopy was conducted and it was discovered that the sling had been placed through the urethral causing an obstruction.The issue is reportedly on going.The patient was referred to a urologist for further consultation.
