(b)(4).The returned stone cone was analyzed, and a visual inspection note that the blue outer sheath was detached from the working length and coil.Additionally, there was a tear and signs of damage at the distal end of the blue sheath.The observed condition would not be able to recapture the coil.No other problems with the device were noted.The reported event was confirmed.Based on all available information, the investigation concluded that it is likely that the difficulty experienced by the user occurred while testing the device prior to use.The damage is consistent with excessive force applied while attempting to sheath the coil.Therefore, the most probable root cause is unintended use error.A labeling review was performed and, from the information available, this device was used per the instructions for use (ifu) / product label.

Event Description

It was reported to boston scientific corporation that a stone cone nitinol retrieval coil was used in the middle and upper ureter during a ureteral lithotomy under rigid ureteroscopy procedure performed on (b)(6) 2021.During the procedure, before the device could be used, the coil of the stone cone could not close again outside of the patient.Another stone cone device was used to complete the procedure.There were no patient complications as a result of this event.The patient's condition at the conclusion of the procedure was reported to be stable.The investigation results revealed that the sheath was torn at the distal section; therefore, this is now an mdr reportable event.
