The olympus field service engineer (fse) replaced the hoses.The fse will complete the repair when a check valve is received.This event is under investigation.A supplemental report will be submitted upon receiving additional information.

Event Description
|
It was reported by an olympus field service engineer (fse) that the hoses in the endoscope reprocessor were degrading.This was causing black specks in the disinfectant.It was reported this was found during preventative maintenance.There was no patient harm or impact to patient care due to this event.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 11 years since the subject device was manufactured.Based on the results of the investigation, the inner wall of the hose inside the equipment was peeling off in fragments, which came out as black foreign material during reprocessing the reporters occupation is director of surgical services.Olympus will continue to monitor the field performance of this device.
