The device was returned, and an initial evaluation was conducted by olympus; however, investigation is ongoing.During the initial evaluation, the user report "front display was acting up" was confirmed.The lamp hours were not displaying on the lamp indicator due to a faulty main printed circuit board.Consequently, the light output measured outside of the specifications.The scope socket condition was ok, the main power switch was up to date, the fan was functioning properly, and the air pressure tested within range.However, there was corrosion noted inside the air tubing.Furthermore, the turret filter was faulty and the lamp will require replacing.If additional information becomes available following device evaluation, a supplemental report will be filed.
 
Event Description
The customer called in to report that the light source on the evis exera iii xenon light source began smoking during a procedure and the image became distorted.According to the initial reporter, they inspected the lamp to see why it was smoking, but it appeared to be fine.The user decided to switch to another tower to successfully complete the procedure.There was no patient harm or consequence reported as a result of this event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the approved final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the definitive root cause of the reported issue could not be determined.Olympus will continue to monitor field performance for this device.
