The product was not returned for evaluation.Without the return of the device, the root cause of the problem cannot be determined.The manufacturing records for this lot were reviewed and did not reveal any outstanding discrepancies, design, or quality concerns.
 
Event Description
The patient was undergoing a thrombectomy procedure in the left anterior descending artery (lad) using an indigo system catrx aspiration catheter (catrx), a penumbra engine (engine), a penumbra engine canister (canister), a non-penumbra rhv and a guidewire (.014 wire).During the procedure, the physician completed the first pass using the catrx.The physician then advanced the catrx to the area of clot for the second pass and turned the aspiration on; however, there was no flow and the catrx was unable to aspirate the thrombus.He also tried to troubleshoot the canister and the engine but found no issue with them.All four lights of the engine were illuminated.Therefore, the physician decided to remove the catrx.It was reported that there was no damage seen on the catrx after it was removed.The procedure was completed using another catrx.There was no report of an adverse effect to the patient.
