The subject device was returned to omsc for evaluation.Omsc checked the subject device and found that there was no abnormality on the exterior and no trace of liquid like blood on the exterior, also the subject device functioned without any problem.The exact cause of the reported event could not be conclusively determined at this time.If additional information becomes available, this report will be supplemented.
 
Event Description
Olympus medical systems corp.(omsc) was informed from the user that when the subject device was stored in the storage after reprocessing, it was confirmed that liquid like blood was dripping from the distal end of the endoscope.The field service engineer visited the facility and was informed from the user that there was possibility that the user had not followed proper reprocess procedures.There was no report of patient injury associated with the event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation and corrections to g2 and h6.G2 - checked "other" to add the country japan.H6 - corrected the component code.A review of the device history record found no deviations that could have caused or contributed to the reported issue.The device met all specifications at the time of shipment.Based on the results of the investigation, it is likely the liquid could not be removed during cleaning, which caused the liquid to flow out of the air supply channel.The specific root cause could not be determined at this time.Olympus will continue to monitor field performance for this device.
