The system was used for treatment.This case is reportable as a mdr due to the reportable malfunction pressure dome membrane leak.Since this reportable malfunction is only associated with the kit, this mdr will only be against the kit.The kit lot number was not provided; therefore, a batch record review could not be performed trends were reviewed for complaint category, pressure dome membrane leak.No trends were detected for this complaint category.The assessment is based on the information available at the time of investigation.No product was returned for investigation; therefore, it could not be determined if the product met specification based solely on the information provided by the customer.The customer stated when attempting to remove the kit the return pressure dome leaked when it was removed.Section 5-43 of the cellex operators manual (b)(4) on treatment complete states "note: to release all internal pressure before removing the kit, always remove all pump tubing segments before removing any pressure domes.Check the screen display to confirm that the pressure has been released." the customer stated they removed the pressure domes first, then the pump tubing segments.The root cause of the pressure dome membrane leak was most likely due to use error during removal of the pressure dome by the end user.No further action is required at this time.This investigation is now complete.(b)(4).
 
Event Description
The customer contacted mallinckrodt to report they experienced a pressure dome membrane leak with their cellex photopheresis kit ("kit") during an extracorporeal photopheresis (ecp) treatment.The customer reported they were trying to push air in the collect line towards the centrifuge, when the pressure dome detached from the pressure sensor.The customer aborted the ecp treatment and did not return residual blood within the kit to the patient.The customer reported the patient was in stable condition.The customer stated when unloading the kit, the return pressure dome ruptured when it was removed.The customer reported when unloading the kit they removed the pressure domes first, and then the pump tubing segments.The customer did not return the product for investigation.
