The device was evaluated by olympus.The evaluation confirmed the reported event of device was not getting enough air.The evaluation uncovered the scope socket was worn out causing poor connection and a worn-out air socket causing the air pressure to leak for low air output.Additionally, the evaluation found the front panel, rear panel and the front panel chassis to be corroded.The non-olympus lamp was over 400 hours which was below specification and required replacement.The faulty parts were replaced.The device was inspected and passed olympus functional standard.The investigation is ongoing; therefore, the root cause of the reported event cannot be determined at this time.However, if additional information becomes available this report will be supplemented accordingly.

Event Description
|
A user facility reported to olympus that during preparation for use, the evis exera ii xenon light source was not getting enough air.Upon inspection and testing of the return device, inappropriate heat sink installation was observed.There was no harm or user injury reported due to the event.

|
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 10 years since the subject device was manufactured.Based on the results of the investigation, it is likely that the heat sink and the lamp base were burnt because a non-olympus lamp was used and excessive insulation applied leading to excessive current.Olympus will continue to monitor the field performance of this device.
