Event Description
It was reported that a male patient underwent a cardiac ablation procedure with a lasso¿ electrophysiology catheter and a device entrapment occurred and patient death.It was reported the lasso¿ electrophysiology catheter got stuck in the patient's mitral valve.The surgery had to be converted to open heart and after one day, the patient died.Multiple attempts have been made to obtain clarification to this complaint.However, no further information has been made available.Should more information become available, it will be reviewed and processed accordingly.

Since no device has been received for analysis, no product investigation can be performed, and the customer complaint cannot be confirmed.Manufacturing record evaluation (mre) cannot be conducted because no lot number was provided by the customer.If additional information is received regarding this event, a supplemental 3500a report will be submitted to the fda.Manufacturer's ref.#(b)(4).

During an internal review on (b)(6) 2023, it was decided to update the previously reported h6.Health effect - clinical code of "appropriate term/code not available (e2402)" which was used to represent "injury (e20)" and updated it with ¿unspecified heart problem (e0623)¿ as this is more representative of the heart injury experienced by the patient.If additional information is received regarding this event, a supplemental 3500a report will be submitted to the fda.Manufacturer's ref.# (b)(4).
