Event Description
Information was received from a manufacturer representative regarding a patient who was implanted with an implantable neurostimulator (ins) for lumbar radiculopathy and spinal pain.It was reported that the patient was having ins replacement today and caller states the physician cut the 37082 extension accidentally.The caller states the physician was going to replace that extension but could not 'get it out'--when the physician pulled the extension he 'sheared' the lead that was connected to the extension.The caller states that in the end they replaced the ins and connected it to the 8 contact lead but the quad leads are now remaining and not connected to the ins.
 
Event Description
Additional information received.The extension was removed but the leads are still implanted without being connected to the battery.Patient was seen in the office yesterday and is doing well.
