It was reported to boston scientific corporation that a solyx blue sis system device was attempted to be implanted into the patient during a solyx single incision sling procedure performed on (b)(6) 2021 for the treatment of stress incontinence.At the end of the procedure, it was found during cystoscopy that the other side of the mesh caught the edge of the urethra which resulted in urethral perforation.It was also reported that the physician experienced difficulty in advancing the device.A urologist was called in to remove the mesh and to suture the small hole in the urethra.The entire mesh was removed but the mesh carrier that was implanted correctly was left inside the patient.Reportedly, the procedure was rescheduled to complete in six months' time.The current condition of the patient is good and expected to be fully recovered.
