Olympus technical assistance center (tac) support spoke to the customer to troubleshoot the device.Customer reported that the device spare lamp lit up when trying to use the device.Tac instructed the customer to replace the lamp since the lamp life was over 300 hours.The customer will replace the lamp and see if it would fix the device issue.Customer later reported that this unit have been retired and is no longer in service.Unit have been traded in for newer equipment in the surgery department.The investigation is ongoing; therefore, the root cause of the reported event cannot be determined at this time.However, if additional information becomes available this report will be supplemented accordingly.
 
Event Description
It was reported to olympus that during preparation for use, the evis exera iii xenon light source spare lamp lit up.There was no patient harm or user injury reported due to the event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's investigation.The device referenced in this report has not been returned to olympus for evaluation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the legal manufacturer's investigation, the root cause of the main lamp not lighting up could not be identified.Olympus will continue to monitor the field performance of this device.
