Event Description
It was reported that at the beginning of the case of a pulmonary vein isolation procedure, it was noticed that the lumen of the intellamap orion high resolution mapping catheter was occluded during the catheter flushing.An attempt was made to send fluid with the injector, but failed.The case was continued with another intellamap orion high resolution mapping catheter.No patient complications were reported.The device is expected to be returned for analysis.

Manufacturer Narrative
Visual examination of the returned intellamap catheter revealed no external abnormalities.A flow test was performed and the device was irrigated and repeatedly had the "po2: occlusion" error.An occlusion could not be found during testing.Based on the information provided, there is no evidence that the device was used in a manner inconsistent with the labelled indications/instructions for use (ifu).The manufacturing batch record review confirmed that the device met all material, assembly and performance specifications.
