The results of the investigation are inconclusive since the reported device was not returned for analysis.Based on the information received, the cause of the reported event could not be conclusively determined.Per the physician, the cause of the pulmonary edema was unknown.(b)(4).
 
Event Description
Orbital atherectomy treatment was selected for the left circumflex artery.The diamondback coronary orbital atherectomy device (oad) was operated on low speed via antegrade and after five treatment passes the oad was able to cross the lesion.The oad was pulled back to the proximal edge of the lesion and a sixth treatment pass was performed, during which the oad stalled.The oad was powered on and the stall issue recurred and it was revealed that the oad was stuck in the vessel.The oad was forcibly removed and imaging revealed a perforation where the crown had become stuck in the vessel.Stent placement was performed to resolve the perforation.The patient also experienced pulmonary edema during the procedure and this was resolved.The patient remained in the hospital for three to four days and was discharged as reportedly fine.
