Event Description

Additional information indicated that during the ablation of the lspv, the ablation could not be stopped immediately, so a double stop was performed. Then the patient became hypotensive, before the effusion was noted. The effusion developed into a tamponade. An emergent sternotomy was performed to repair the perforation. The patient then went into cardiac arrest with ventricular fibrillation.Resuscitation attempts were taken to no avail. The patient died on the operating table.
