Event Description
It was reported that before use, the svo2 calibration could not be performed.It was unknown what type of monitor was used and if any error message was displayed on the monitor.There was no injury or complications to the patient.During the product evaluation, it was revealed that the balloon did not inflate due to leakage from a tear.The torn edges were not able to match up.
 
Manufacturer Narrative
One 777f8 catheter without any attached components was returned for examination.The reported issue of the "svo2 calibration could not be performed" was unable to be confirmed.The catheter passed the in-vitro calibration on a vigilance ii monitor.All through lumens were patent without any leakage or occlusion.The balloon did not inflate due to leakage from a tear, approximately 1.5 mm x 0.5 mm in size, along the distal balloon bonding site.The torn edges were not able to match up.There was no visible damage observed from catheter body.The sample was sent for further evaluation.The catheter passed the attenuation test.A review of the manufacturing records indicated that the product met specifications upon release.As part of the manufacturing process, 100% balloon inflation and visual inspection is performed.A product risk assessment was initiated to address the failure complaint histories for all reported events are reviewed against trending control limits on a monthly basis, and any excursions above the control limits are assessed and documented as part of this monthly review.
