This follow-up report is being submitted to relay additional information.The following sections were updated/corrected updated: b4, b5, d4, d9, g3, g6, h2, h3, h4, h6, h10.Visual examination of the returned product identified the hex tip to be fractured.Wear rings are present around the shaft.The handle grip is scuffed and has a single discoloration spot.The connector is also scuffed.Additionally the features of the fracture surface visually align with a torsional overload fracture failure.Device history record was reviewed and no discrepancies relevant to the reported event were found.A definitive root cause cannot be determined.If any further information is found which would change or alter any conclusions or information, a supplemental will be filed accordingly.Zimmer biomet will continue to monitor for trends.
