|
Customer was having an issue with the device disinfectant (lcg cartridge) drawer closing.They were able to correct the problem, possible tray alignment, and finish the process with no error codes.There is no patient involvement and no harm reported to any patient.

Field service engineer (fse) went on site to evaluate the customer reported issue of having trouble closing the lcg (disinfectant) drawer.Customer informed the fse that the drawer will not open or close correctly.Fse inquired if they were pulling off the stickers before loading the bottles in the drawer.Customer reported that it was being done.The drawer tracks and draw flow on the rails was were functioning normally.However, upon opening the drawer, fse observed that the stickers were still on the bottle.Fse explained to the customer proper process of loading bottles and how to close the drawer once by sliding up to the cutters before pushing draw closed with slight force to cut in to caps.They will follow the proper process of sticker removal and proper loading as well.No repair was required.Evaluation is ongoing.Supplemental report(s) will be submitted when any relevant new information is available.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, it is likely the disinfectant bottle drawer was not fully pushed in when replacing the disinfectant bottle.Olympus will continue to monitor field performance for this device.
